Membership Cancellation Request

If you wish to change or cancel your membership with CrossFit Moncton we ask that you fill out this form least 7 days prior to your renewal date so we can remove your membership from our system well before the next payment is due. If less than a 7 days notice is given we cannot guarantee the next payment will not be processed and cancellation will be pushed to the next renewal date. We do not offer holds, unless for medical reasons.

1. If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.

2. Your membership will be canceled 7 days from the submission of the form below. Note that if you have a scheduled renewal payment within this 7-day period, the payment will be processed as scheduled. All payments are non-refundable.

3. Agreements are auto renewed unless the MEMBERSHIP CANCELLATION FORM is submitted to us at least 7 days prior to your scheduled auto renewal.

4. If you’re sure you’d like to cancel, just complete and submit the form below 7 days prior to your next scheduled payment. This will serve as your 7-day written cancelation notice as required by your membership agreement.
